Background

  • Caused by transfusion of platelet antigen positive blood product to a patient who is antigen negative
    • Most commonly HPA-1a positive product to an HPA-1a negative patient

Clinical Manifestations

  • Onset about 9 days after transfusion (range 1 to 24 days)
  • Severe thrombocytopenia, usually below 10

Diagnosis

  • Testing patient for platelet specific antibodies

Management

  • IVIg 1 g/kg daily for 2 days
    • Response usually seen by 4 days
