Post-transfusion purpura

Background
- Caused by transfusion of platelet antigen positive blood product to a patient who is antigen negative
- Most commonly HPA-1a positive product to an HPA-1a negative patient
Clinical Manifestations
- Onset about 9 days after transfusion (range 1 to 24 days)
- Severe thrombocytopenia, usually below 10
Diagnosis
- Testing patient for platelet specific antibodies
Management
- IVIg 1 g/kg daily for 2 days
- Response usually seen by 4 days
