Pediatric Dosing

Neonatal HIV prophylaxis

• ≥35 weeks gestation at birth: 4 mg/kg PO q12h for 6 weeks
• 30 to 34+6 weeks: 2 mg/kg PO q12h for the first 2 weeks, then 3 mg/kg PO q12h for another 4 weeks
• <30 weeks: 2 mg/kg PO q12h for the first 4 weeks, then 3 mg/kg PO q12h for another 2 weeks
• IV can be given at 75% of the oral dose q12h
