Mechanical valves

  • MVR: Warfarin 2.5-3.5 with asa
  • AVR with risk factors (atrial fibrillation, thromboembolism, LV dysfunction, hypercoagulable state): Warfarin 2.5-3.5 with ASA
  • AVR without risk factors: Warfarin 2-3 with ASA

Tissue valves

  • MVR: Warfarin 2-3 for three months, then ASA
  • AVR: Warfarin 2-3 for three months, then ASA
  • TAVR: Clopidogrel and ASA for 6 months

Bridging

  • Mechanical aortic valve undergoing minor procedure, continue warfarin
  • Invasive or major surgery
  • Bileaflet mAVR with no risk factors, no bridging
  • mAVR with risk factors, bridge with UFH/LMWH
  • mMVR, bridge with UFH/LMWH
