Mechanical valves
- MVR: Warfarin 2.5-3.5 with asa
- AVR with risk factors (atrial fibrillation, thromboembolism, LV dysfunction, hypercoagulable state): Warfarin 2.5-3.5 with ASA
- AVR without risk factors: Warfarin 2-3 with ASA
Tissue valves
- MVR: Warfarin 2-3 for three months, then ASA
- AVR: Warfarin 2-3 for three months, then ASA
- TAVR: Clopidogrel and ASA for 6 months
Bridging
- Mechanical aortic valve undergoing minor procedure, continue warfarin
- Invasive or major surgery
- Bileaflet mAVR with no risk factors, no bridging
- mAVR with risk factors, bridge with UFH/LMWH
- mMVR, bridge with UFH/LMWH
