Mandatory Reporting

  • Mandatory reporting for specific medical conditions, functional, and visual impairments
  • MOT form includes:
    • Cognitive impairment that results in substantial limitation in their ADLs
      • Dementia
        • Frontotemporal dementia must be reported
        • Lewy-body dementia must be reported
        • Impairment in 1 ADL or 2 iADLs must be reported
      • Brain injury
      • Unknown, or other
    • Discretionary report
      • The patient has a medical condition, functional impairment, or visual impairment that may make it dangerous for the person to operate a car
      • Doesn't take effect immediately, but it will prompt a review by the MOT

Transportation Plan

  • Identify activities that they used to drive to
  • Discuss transportation alternatives
    • Walking
    • Public transportation
    • Taxi accounts (can be cheaper than a car)
