Dosing

  • HIV treatment: 150 mg p.o. twice daily or 300 mg p.o. once daily
  • Hepatitis B: 100 mg p.o. once daily

Renal Dosing

  • GFR >=50: usual dosing
  • GFR 30-49: 150 mg p.o. once daily.
  • GFR 15-29: 150 mg p.o. on day 1 followed by 100 mg p.o. once daily
  • GFR 5-14: 150 mg p.o. on daily 1 followed by 50 mg p.o. once daily
  • GFR <5: 50 mg p.o. on day 1 followed by 25 mg p.o. once daily
  • Hemodialysis: 50 mg p.o. once followd by 25 mg p.o. once daily post dialysis
  • Peritoneal dialysis: 50 mg p.o. once followed by 25 mg p.o. once daily

Pediatric Dosing

Neonatal HIV prophylaxis

  • ≥32 weeks gestation at birth: 2 mg/kg PO q12h until age 4 weeks, then 4 mg/kg (max 150 mg) PO q12h
