Background
- Biliary obstructive from infection-related strictures of the biliary tract in the context of advanced HIV infection
Etiologies
- Cryptosporidium parvum (most common)
- CMV (second-most common)
- Others: Microsporidium, Cyclospora cayetanensis, Isospora, Giardia, Histoplasma
- Idiopathic (also common)
Risk Factors
- Much more common with CD4 less than 100
Clinical Manifestations
- Right upper quadrant pain
- Elevated cholestatic liver enzymes (ALP of 700-800), often with normal ALT and bilirubin
- May have fever and jaundice
- Can result in sclerosing cholangitis and liver failure
