Transfusion-associated hypotension

Background
- Thought to be caused by activation of bradykinin
- More common with platelet transfusions
Clinical Manifestations
- Decrease in systolic or diastolic BP by 30 mmHg or more
- Usually within 15 minutes of transfusion starting
- May have nausea, vomiting, urticaria, or dyspnea
- Self-resolves after 30 minutes to 3 hours
Management
- Stop the transfusion, and do not restart
- Supportive care, including IV fluids
- Rule out TRALI and severe allergic reactions
