Fibrinogen

- Available as concentrate or cryoprecipitate
- Concentrate
- Dose is 4 g, equivalent to 10 units of cryoprecipitate
- Should raise fibrinogen by about 0.9 g/L
- Cryoprecipitate
- Each unit has a volume of 10 to 15 mL
- Dose is 0.1 unit/kg, but typically dispensed as 10 units
- Infused over 15 to 30 minutes per dose (maximum 3.5 hours)
- Each dose should raise fibrinogen by about 0.7 g/L
Indications
- Active bleeding, with fibrinogen less than 1 g/L
- Massive hemorrhage, with fibrinogen less than 1.5 to 2 g/L
- Intracranial hemorrhage related to rissue plasminogen activator, with fibrinogen less than 2 g/L
- Emergency, with clinical status suggestive of low fibrinogen (no time to check fibrinogen)
- Obstetrical hemorrhage
- Hemophilia, when other factor concentrates are unavailable
