Calcium channel blocker toxicity

Pathophysiology

  • Blocks L-type calcium channels, including in pancreas.

Clinical Manifestations

  • Hypotension and bradycardia
  • Hyperglycemia from effect on pancreas (which differentiates from beta-blocker overdose that is often hypoglycemic)

Management

  • Intubate early
  • Call the Poison Centre
  • High dose insulin euglycemic therapy
    • Humulin R at 0.5-1 U/kg/hr
  • Can also try intralipid
  • Can also try atropine for bradycardia, sodium bicarb
