Fomepizole

Procedure

1. Loading dose of 15 mg/kg IV
2. Start maintenance at 10 mg/kg IV q12h for 48h
3. Incrase to 15 mg/kg IV q12h, and continue until:
   • Toxic alcohol below toxicity level
   • Normal plasma pH
   • Clinical improvement
4. If ongoing dialysis, increase dose to 15 mg/kg IV q4h
