Elimination

• Catheter prn for discomfort from urinary retention or incontinence
• Fleet enema PR daily prn

Pain/Dyspnea

• Oxygen 1-6L NP prn
• Opioid-naive
  • Hydromorphone 0.2mg sc q30min prn
  • Morphine 1mg sc q30min prn
• On opioid, convert to
  • Hydromorphone ___mg sc q4h standing with [10% TDD] mg q30min prn
  • Morphine ___mg sc q4h standing with [10% TDD] mg q30min prn

Restlessness/Agitation

• Haloperidol 0.5mg sc q4h prn

Secretions

• Glycopyrrolate 0.4mg sc q4h prn

Fever

• Acetaminophen 650mg PR q4h prn

Seizures

• Lorazepam 2mg sc STAT x1 dose prn seizure, call MRP

Oral and Eye Care

• Oral Balance Gel for mouth care QID standing and q1h prn
• Tears Naturale 1-2 drops both eyes QID standing and q2h prn, if eyes not closed
