Calcium channel blocker toxicity

Pathophysiology
- Blocks L-type calcium channels, including in pancreas.
Clinical Presentation
- Hypotension and bradycardia
- Hyperglycemia from effect on pancreas (which differentiates from beta-blocker overdose that is often hypoglycemic)
Management
- Intubate early
- Call the Poison Centre
- High dose insulin euglycemic therapy
- Humulin R at 0.5-1 U/kg/hr
- Can also try intralipid
- Can also try atropine for bradycardia, sodium bicarb
