Primary sclerosing cholangitis

Background
- Chronic disorder characterized by inflammation of the intra- and extra-hepatic bile ducts
- Commonly comorbid with ulcerative colitis (about 70% of cases)
- Possibly a hereditary component
Clinical Manifestations
- Often asymptomatic, with persistent ALP elevation
- Fatigue, pruritus, jaundice
- Increased risk of hepatocellular carcinoma
Investigations
- Anti-smooth muscle antibodies; may also have positive ANA, RF, and p-ANCA
