Periorbital cellulitis

Management

  • Need to distinguish preseptal from orbital cellulitis: preseptal will have swelling, may have pain and tenderness, fever, and luekocytosis, but should not have painful eye movements, proptosis, ophthalmoplegia, or vision impairment
  • CT sinus/orbit with contrast
  • Antibiotics that cover Gram-positives and Gram-negatives, including Pseudomonas
  • Duration 3 to 4 weeks, which should include an initial 1 to 2 weeks of IV
