Dosing
- HIV treatment: 150 mg p.o. twice daily or 300 mg p.o. once daily
- Hepatitis B: 100 mg p.o. once daily
Renal Dosing
- GFR >=50: usual dosing
- GFR 30-49: 150 mg p.o. once daily.
- GFR 15-29: 150 mg p.o. on day 1 followed by 100 mg p.o. once daily
- GFR 5-14: 150 mg p.o. on daily 1 followed by 50 mg p.o. once daily
- GFR <5: 50 mg p.o. on day 1 followed by 25 mg p.o. once daily
- Hemodialysis: 50 mg p.o. once followd by 25 mg p.o. once daily post dialysis
- Peritoneal dialysis: 50 mg p.o. once followed by 25 mg p.o. once daily
