Background

  • Thought to be caused by activation of bradykinin
  • More common with platelet transfusions

Clinical Manifestations

  • Decrease in systolic or diastolic BP by 30 mmHg or more
    • Usually within 15 minutes of transfusion starting
  • May have nausea, vomiting, urticaria, or dyspnea
  • Self-resolves after 30 minutes to 3 hours

Management

  • Stop the transfusion, and do not restart
  • Supportive care, including IV fluids
  • Rule out TRALI and severe allergic reactions
