- Anaphylactoid reaction to vancomycin characterized by a diffuse pruritic rash
- Can also have hypotension, nausea & vomiting, fevers, and even angioedema
- Caused by direct activation of mast cells and basophils, releasing histamine
- Treated by switching to prolonged infusion
