Procedure
- Loading dose of 15 mg/kg IV
- Start maintenance at 10 mg/kg IV q12h for 48h
- Incrase to 15 mg/kg IV q12h, and continue until:
- Toxic alcohol below toxicity level
- Normal plasma pH
- Clinical improvement
- If ongoing dialysis, increase dose to 15 mg/kg IV q4h
