- Wean, screen, and extubate
- Should have protocol for daily screening for SBT
- Readiness for SBT defined as
- Cause of respiratory failure has improved
- Patient oxygenating adequately: PaO
2:FiO2ratio ≥150 or SpO2≥90% while on FiO2≤40% and PEEP ≤5 cmH2O - pH >7.25
- Not on pressors (or low pressors)
- Able to initiate inspiratory effort
- If above criteria are met, should initiate an SBT
