Therapeutic Drug Monitoring
- Measure peak and trough within 72 hours of starting or adjusting dose, adjusting interacting medications, uncertain adherence, or signs of toxicity
- Peak measured 2 hours after a dose
- Target trough 20-40 mg/L and peak <100 mg/L
