Dosing
- Oral: 500 mg/125 mg p.o. three times daily, or 875 mg/125 mg p.o. twice daily
- Intravenous:
- Usual dose: 1 g (1.2 g) IV every 8 hours
- High dose: 2 g (2.2 g) IV every 8 to 12 hours
