* Sleep disordered breathing&lt;br /&gt;

** Non-obstructive sleep hypoventilation (10%), defined as either&lt;br /&gt;
*** PaCO&amp;lt;sub&amp;gt;2&amp;lt;/sub&amp;gt; (or surrogate such as ETCO&amp;lt;sub&amp;gt;2&amp;lt;/sub&amp;gt; or transcutaneous CO&amp;lt;sub&amp;gt;2&amp;lt;/sub&amp;gt;) &amp;gt;55 mmHg for &amp;gt;10 min, or&lt;br /&gt;
*** An increase in PaCO&amp;lt;sub&amp;gt;2&amp;lt;/sub&amp;gt; (or surrogate) &amp;gt;10 mmHg compared to an awake supine value to a value &amp;gt;50 mmHg for &amp;gt;10 min&lt;br /&gt;
