Wellens syndrome

Identification

  • Deep, wide, T-wave inversions in V2-V3 (and often involved V1-V4)
  • Type A: biphasic, with initial positivity & terminal negativity (25% of cases)
  • Type B: deeply and symmetrically inverted (75% of cases)

Interpretation

  • High risk for high-grade left anterior descending (LAD) artery lesions
