| I | Acute bacterial prostatitis | Symptoms of UTI with bacteriuria |
| II | Chronic bacterial prostatitis | Recurrent episodes of UTI with same organism |
| III | Chronic prostatitis/chronic pelvic pain syndrome | 90% of cases; symptoms primarily of pain with the exclusion of other causes |
| IIIA | Inflammatory | Leukocytes present in prostatic secretions or semen |
| IIIB | Non-inflammatory | Leukocytes not present |
| IV | Asymptomatic inflammatory prostatitis | Asymptomatic and discovered incidentally on biopsy or other investigation |
