Management
- Anticoagulation
- If CVAD still needed: continue anticoagulation while it is in situ
- If CVAD no longer needed:
- If it can be left in situ, leave it for up to 3-5 days, up to 7 days, while on anticoagulation to reduce risk of embolism
- Continue anticoagulation for 3 months
- Choice of anticoagulation is primarily LMWH
Prevention
- Use small calibre catheter possible
- Ensure proper catheter tip location
- Remove CVADs when no longer needed
- No benefit to prophylactic anticoagulation
