Therapeutic Drug Monitoring

• Measure peak and trough within 72 hours of starting or adjusting dose, adjusting interacting medications, uncertain adherence, or signs of toxicity
  • Peak measured 2 hours after a dose
• Target trough 20-40 mg/L and peak <100 mg/L

Adverse Drug Reactions

• Neutropenia correlates well with levels
