Fibrinogen

  • Available as concentrate or cryoprecipitate
  • Concentrate
    • Dose is 4 g, equivalent to 10 units of cryoprecipitate
    • Should raise fibrinogen by about 0.9 g/L
  • Cryoprecipitate
    • Each unit has a volume of 10 to 15 mL
    • Dose is 0.1 unit/kg, but typically dispensed as 10 units
    • Infused over 15 to 30 minutes per dose (maximum 3.5 hours)
    • Each dose should raise fibrinogen by about 0.7 g/L

Indications

  • Active bleeding, with fibrinogen less than 1 g/L
  • Massive hemorrhage, with fibrinogen less than 1.5 to 2 g/L
  • Intracranial hemorrhage related to rissue plasminogen activator, with fibrinogen less than 2 g/L
  • Emergency, with clinical status suggestive of low fibrinogen (no time to check fibrinogen)
  • Obstetrical hemorrhage
  • Hemophilia, when other factor concentrates are unavailable
