Dosing
- DVT/PE prophylaxis: 40 mg/kg SC q24h (increased to q12h if BMI ≥40)
- DVT/PE treatment: 1 mg/kg SC q12h, or 1.5 mg/kg SC q24h (use q12h dose if BMI ≥40)
