Mandatory Reporting
- Mandatory reporting for specific medical conditions, functional, and visual impairments
- MOT form includes:
- Cognitive impairment that results in substantial limitation in their ADLs
- Dementia
- Frontotemporal dementia must be reported
- Lewy-body dementia must be reported
- Impairment in 1 ADL or 2 iADLs must be reported
- Brain injury
- Unknown, or other
- Dementia
- Discretionary report
- The patient has a medical condition, functional impairment, or visual impairment that may make it dangerous for the person to operate a car
- Doesn't take effect immediately, but it will prompt a review by the MOT
- Cognitive impairment that results in substantial limitation in their ADLs
Transportation Plan
- Identify activities that they used to drive to
- Discuss transportation alternatives
- Walking
- Public transportation
- Taxi accounts (can be cheaper than a car)
