Aconite poisoning

- Toxic alkaloid derived from monkshood, wolfsbane, and other members of the genus Aconitum
- Bins and opens sodium channels, preventing their inactivation
- Manifestations include GI (nausea, vomiting, cramping), neurologic (numbness, particularly of face and mouth then progressing to limbs, muscle weakness, and coma), and cardiac (hypotension, arrhythmias)
- Management is supportive
- Bradycardia: atropine
- Long QT/TdP: magniseium
- Arrhythmias: cardioversion
- ECMO
